胆嚢病変に対する造影3次元超音波検査の有用性

三次元造影超音波検査(3D CEUS)は、高い空間分解能や客観性、リアルタイム性などの特徴を有しており、胆嚢病変の鑑別診断に有用である。本稿では、胆泥、非腫瘍性ポリープ、慢性胆嚢炎、胆嚢癌の鑑別について、3D CEUSで得られる特徴的な造影所見を報告する。
Three-dimensional contrast-enhanced ultrasonography (3D CEUS) is useful for differentiating gallbladder disease due to the advantages of high spatial resolution, objectivity, and real-time scanning. We report the specific images of biliary sludge, benign gallbladder polyp, chronic cholecystitis, and gallbladder carcinoma, by using 3D CEUS. All the biliary sludge showed avascular lesion. All the benign gallbladder polyps showed pedunculated polyps with a thin vessel in a stalk. Typical finding of chronic cholecystitis was homogeneous enhancement in a thick-wall. Typical finding of gallbladder carcinoma was heterogeneous enhancement with irregular vessels in sessile polyps or thick-walls.
