Poorman’s Outback

浅大腿動脈の慢性完全閉塞（CTO）病変に対する血管内治療（EVT）においては、順行性に進めたガイドワイヤーが線維性被膜に阻まれて遠位部真腔に再突入できず、内膜下に進んでしまうことがしばしば認められる。この様な状況において、体表から進めた18G金属針を用いることでガイドワイヤーを真腔に導く手法としてPoorman’s Outback法を考案した。
In the endovascular therapy for the cases with chronic total occlusion of superficial femoral artery(SFA), we sometimes face the situation where a guidewire fails to enter the distal true lumen of SFA, and advance into the sub-intimal space beyond the occluded segment. I invented Poorman’s Outback method as a low-cost and straightforward technique to lead a guidewire to the distal true lumen by using a 18G metal needle which is advanced from body surface.
