Elastgraphyと肝実質灌流血の検討

慢性肝疾患の診療に際して近年、硬さ評価・線維化診断のためにElastgraphyが広く用いられるようになった。しかし、肝の硬さを規定する因子についてはいまだ不明な点が多く、うっ血など血流動態も影響するとされているが明らかでない。そこで今回我々は、ソナゾイド造影超音波を用い、肝実質灌流動態の変化が硬さに影響するか検討した。結果、肝実質灌流の動脈化が進行すると硬さも上昇する傾向があり、肝実質灌流動態は線維化以外の硬さの規定因子の一つとなりうることが示唆された。
Evaluating the liver fibrosis by Virtual Touch Quantification (VTQ) is important to know the prognosisof a patient with chronic liver disease. However, except for the liver fibrosis, the factors affecting VTQare still not clear. So that, we examined whether liver blood flow in the perfusion parametric imaging(PPI) using Sonazoid® enhanced ultrasound would participate in VTQ.
