医学的判断の「内容」に対する情報システムの支援と介入
〜 画像検査オーダ時のClinical Decision Support 使用が米国で義務化 〜

米国で2017年1月以降にCT・MRI・核医学といった高額な画像検査を、公的保険であるMedicareに加入して
いる外来患者に対してオーダする際には、オーダリングシステムに組み込まれたClinical Decision Support（臨
床診断意思決定支援：CDS）と呼ばれる仕組みを使用して、文献根拠に準拠して定義された政府認証済の検査適
合度一覧表に検査目的を照会することが義務づけられる。この記事では、その様なCDSに関する論文や議論を紹
介する。
The Protecting Access to Medicare Act (PAMA) of 2014 passed the United States Congress on April
1, 2014. It mandates that physicians ordering advanced diagnostic imaging services (CT, MRI, nuclear medicine and PET) for Medicare outpatients must consult government-approved, evidence-based appropriate-use criteria, through a CDS system after January 1, 2017. Thus such CDS systems should be integrated with computerized physician/provider order entry systems in the physician office, hospital outpatient, and emergency department settings before enforcement of the law. This article introduces publications and discussions regarding the CDS system.
