現代の乳癌治療は過剰か？

乳癌センチネルリンパ節生検(SLN)について扱った3本の論文をご紹介したい。まず、ITCs (isolated tumor cells)について検討したKeruakous ARらの論文（文献1）では、ITCsの存在は、治療成績に影響を与えず、ITCsの存在自体で、追加治療を決定する必要はない、とする。つづいて、van der Looらは、micrometastasesを有する症例の無病生存率は依然として高く、全例に補助化学療法は推奨できない、と述べ（文献2）、Co Mらの論文（文献3）でも、ITCsに対する腋窩郭清は必要ないことをあらためて裏付けている。以上の論文は、薬物療法を中心とした過剰治療の時代に警鐘をならしているものと解釈される。

We have reported the 3 recent articles which dealt with the significance of isolated tumor cells (ITCs) or micrometastases in sentinel lymph node biopsy (SLNB) in patients with breast cancer.
They documented that overtreatment should be avoided basically for these patients but further studies would be required to obtain the final conclusions.
