─僧帽弁閉鎖不全症に対する経カテーテル低侵襲治療の可能性─

MitraClip®は、経カテーテル僧帽弁形成術のデバイスとして欧米で最も多く用いられている治療機器である。これまで手術への忍容性がないと判断され心臓外科手術を見送られてきた重症僧帽弁閉鎖不全症症例にとってMitraClip®は新たな治療オプションとして大きく期待されている。手技は大腿静脈アプローチで行われ、外科手術と比較して極めて低侵襲であるが、手技のほとんどが経食道心エコー図ガイドで行われる点などから技術的には容易ではない。本稿ではMitraClip®の治療概念と臨床的有用性、手技の実際について概説する。
Percutaneous edge-to-edge mitral valve repair using the MitraClip® has emerged as a novel option for the treatment of severe mitral regurgitation patients with high conventional open-heart surgical risk. MitraClip® is inserted into the femoral vein and therefore this procedure is further less invasive than conventional open-heart surgery. However, unlike other catheter interventions, most of the MitraClip® procedures are performed under transesophageal echocardiography guidance. Hence, this procedure requires the specific knowledge and the enough experience. In this article, I will overview the concept, the clinical significance, and the procedures of this innovative device.
