Zone 0-2 TEVARの新たなデバイス:Gore TAG Thoracic Branch Endoprosthesis

胸部ステントグラフト内挿術(TEVAR)は、下行大動脈瘤に対する治療の第1選択として確立したが、弓部瘤に対し
ては外科的バイパス術併用など様々な工夫が必要である。Gore TAG Thoracic Branch Endoprosthesis(TBE)は弓
部1分枝の温存に対応した新たなデバイスであり、市販化に向けて米国で治験が行われている。本稿ではGore TAG
TBEについて、実症例を交えながら紹介する。
Although thoracic endovascular aortic repair (TEVAR) has been accepted as a first line treatment for the
descending aortic aneurysm, TEVAR for aortic arch is still being challenged and requires hybrid treatment,
chimney technique or in-situ fenestration to maintain branch vessel perfusion.
Gore TAG Thoracic Branch Endoprosthesis (TBE) could solve this problem, and an industry-sponsored
US feasibility multicenter clinical trial is ongoing. I introduce this novel device with a representative
clinical case.
