乳腺濃度を考慮した検診について
〜マンモグラフィ・超音波検査分離併用 独立判定の経験から〜

栃木県保健衛生事業団では平成12年度から原則40歳以上の住民を対象にマンモグラフィ・超音波検査分離併
用独立判定による乳がん検診を実施している。各検査方法それぞれの過去画像比較により、併用検診全体の精度
は要精検率約7.4％、がん発見率約0.30％、陽性反応適中度約4.1％であった。今後は独立判定から総合判定に
変更することでより精度を改善できると考えている。
We offer a breast cancer screening program with a combination of mammography and ultrasonography
in Tochigi Public Health Service Association since 2000 to women over 40 years old.
Ultrasonography scanning is performed without knowledge of mammography findings,and interpretation
of mammography and ultrasonography is performed independently. Final decision is made according to
the higher assessment category of the two modalities.
In this article, we report our recent performances from April 2011 to March 2014. Our recall rate is
approximately 7.4%, cancer detection rate is approximately 0.30% and positive predictive value is
approximately 4.1%. In the future these rates will be improved by changing the current independent
decision making to combined assessment of mammography and ultrasonography findings.
