●ABSTRACT
Chemoembolization is the standard of care for unresectable/unablatable HCC in patients with preserved liver function, based on two randomized trials demonstrating improved survival compared to best supportive care. Conventionally, chemoembolization has been performed using a drug emulsion with Lipiodol, ever since observations in the 1980’s and 1990’s that Lipiodol is selectively taken up and retained in liver tumors, that it serves as a drug carrier for local tumor-specific delivery, and that Lipiodol retention is an imaging biomarker for both pathologic response and for survival.Bland embolization with microparticles alone, which has been done since the 1960’s for hepatic tumors, was largely supplanted by chemoembolization. The randomized trial by Llovet which established the survival benefit from chemoembolization also had a bland embolization arm with nearly identical results, falling just shy of statistical significance when the trial was closed. Survival data for HCC patients in the United States treated with bland embolization is indistinguishable from that reported for chemoembolization in a similar US population.Drug-eluting embolic beads are a new platform for intra-hepatic drug delivery with better local drug retention and decreased systemic toxicities compared to Lipiodol chemoembolization. Improvement in cancer outcomes for HCC have yet to be demonstrated. Beads canbe loaded with irinitecan to treat metaststic colon cancer, with encouraging ongoing trials suggesting additive benefit over systemic chemotherapy alone.Radioembolization with yttrium-90 microspheres is a less toxic therapy than chemoembolization with similar oncologic outcomes in HCC and metastatic disease. There are no comaprative trials of radioembolization andchemoembolization. Studies of combinations of radioembolization with systemic chemotherapy for metastatic disease are underway.
