RSNA2014 「おすすめ演題」について
堀川雅弘先生（Dotter Interventional Institute/クオリティラドIVR）にお答えいただきました。
RSNAはIVRを専門にする者としては新しいものに欠ける印象があるが、参加するメリットはその演題・教育講演内容の幅広さにあると思う。Scientificな発表も一部含むが、極力subspecialtyの専門学会では得られにくい内容に注目したい。

PURPOSE: The aim of this retrospective study is to compare the localcontrol effects of microballoon-occluded transarterial chemoembolization (B-TACE) with miriplatin (MPT) and those of conventional TACE with epirubicin (EPIR) for hepatocellular carcinoma (HCC).

日本では既にお馴染みとなった感のあるB-TACE。2014年12月現在、体幹用のmicroballoonカテーテルが使えるのは日本のみであり、そのような先行性をしっかりと活かしている演題。海外では使用できず、今後も恐らく使用される事はなさそうなミリプラチンに対するaudienceの反応が気になるところ。

欧米ではRobotic catheterが既に市場に出てきているが、これはRobotic armを使ったCTガイド下生検の実臨床の報告。10年来話題にされている内容だけれども実際に市場に出回らないのは基本的には面倒だったり実際に困っていないからというのが本音だろう。ただし、使われているMAXIOというインドの製品のパンフレットを見るとかなり良さそうに見えるので密かに注目(http://www.perfinthealthcare.com/Brochure/MAXIO_Brochure.pdf)。数学とコンピューターに強い国インドから。

診断医の力は知識や経験は勿論、文献やインターネットを用いていかにdecision makingを行うか、更にはそれをどのように実際の患者ケアに反映させるかまでの総合力であると思うが、日進月歩の情報社会ではそのための情報収集方法やtoolのupdataが出来るかどうかが死活問題になる。

Hot Topic Session: Meeting the Demand for 24/7 Coverage in Academic Medical Centers

PURPOSE: Many radiology departments are actively considering different methods to best expand to 24/7 attending coverage. In this session, three very different solutions will be explored, with particular attention to the issues relevant to academic medical centers, including: staffing, scheduling, and sustainability; expertise and scope of practice; the academic mission including academic productivity and implications to trainee education and autonomy; and financial considerations. Each model will be discussed, followed by a panel session for open discussion.

アメリカでは放射線科の人数も多く、訴訟大国でもあるため夜間読影のシステムも多彩に発達している。近い将来日本でも見て見ぬふりは出来なくなると思うが、米国と比べるとマンパワーの不足という絶対的なfactorがある。本sessionでは3通りのsolutionが提示されるようであるが、それぞれのメリット・デメリットを考えて将来日本に上手く当てはまる方法を考える一助になれば面白い。

個人的に、アメリカの教育システムは一定レベル以上の人間を多数輩出する方法としては世界一だと思っている。そのコアとなるのがカリキュラム作りと資格試験で、その質をいかに担保するか。そして現在のみならず将来のニーズから逆算する形で世の中の先手を打ったプログラムを作るのが理想。2016年から始まるIVRを放射線診断科と並立のIR(interventional radiology)/DR(diagnostic radiology) double certificate residencyプログラムの動向を含め、日本の放射線科教育の将来を考えるには他国の制度を外から眺めるのは良い機会ではなかろうか。

マーケットが過熱している領域では新しいdeviceが一気に複数出て来る事があるが、aortic stent graftの欧米での現状はまさにそういう感じ。企業主導の宣伝sessionでない場所で複数のdeviceのメリット・デメリットを分析出来ると有り難い。

Focused ultrasoundは前立腺癌以外でも既に研究レベルを超えて実用レベルにある。治療計画は放射線治療医の仕事に似ているようにも見えるが、toolとしてはIVRの領域とも言える。

放射線科の中で経営management, 経済や医療政策に通じる人材を育てる方法を取り上げたsession。RSNAの懐の深さ、戦略性を思い知らされるsession。

Drug eluting deviceの基礎実験。日本人としてはそんなの動注ポートがあるだろと言いたい気持ちはあるが、何が新しいコンセプトなのかは非常に興味を駆られる。
